Request To provide us with the Health Records for my Children<\/h2>\n\n\n\n

Our institution can only issue an extract from medical records to the legal representative?<\/em><\/strong><\/h4>\n\n\n\n

Submission of a new request to the Administration of the Health Care Institution \u201cCity Children\u2019s Clinic No. 13\u201d to provide us with a health record of the health status of my children, submitted by my Attorney, Mr. A.K.<\/p>\n\n\n\n

EXCERPTS OF THE APPEAL\/REQUEST:<\/strong><\/h4>\n\n\n\n

On July 07, 2020, we received a response (ref. No. 06-04\/\u043a-15) signed by the Acting Chief Medical Officer of the Health Care Institution \u201cCity Children\u2019s Clinic No. 13\u201d N.P. Pivovarova stated that\u00a0\u201cdue to the fact that the legal representative of Alexandra Cheropoulou, born on April 01, 2015, is her mother (Liudmila Trofimovich), our institution can issue an extract from medical records only at her request\u201d.<\/em><\/strong><\/p>\n\n\n\n

This decision was appealed as illegal on July 08, 2020, to the Health Committee of the Minsk City Executive Committee (hereinafter \u2013 the Committee).<\/strong><\/p>\n\n\n\n

On July 31, 2020, the Law Office received a response signed by the First Deputy Chairman of the Committee, D.V. Cherednichenko, stating that the facts of violation of the current legislation on healthcare, marriage, and family by the employees of the healthcare institution have been confirmed, and the Head of your institution have been instructed to comply with regulatory acts and to prevent such violation in further work.<\/em><\/p>\n\n\n\n

In such circumstances, we ask you to consider our initial appeal on the merits and provide information about the state of health (extracted from medical records) of Alexandra Cheropoulou, born on April 01, 2015, the daughter of Nicolaos Cheropoulos.<\/strong><\/em><\/p>\n\n\n\n
