We received an answer from the Healthcare Institution \u201cCity Children\u2019s Clinic No. 13\u201d with the rejection of my Attorneys’ request to have an extract of the medical record of my daughter A.C., who was hospitalized there with an \u201cINFECTION\u201d  (COVID-19) in May 2020.<\/em><\/p>\n<\/blockquote>\n\n\n\n

Having considered your appeal regarding the provision of information on the state of health (extracts from medical records) of Alexandra Cheropoulou, born on April 01, 2015, we hereby inform you of the following.<\/strong><\/em><\/p>\n\n\n\n

To obtain an extract from the medical records concerning the state of health of Alexandra Cheropoulou, you need to submit to the Health Care Institution \u201cCity Children\u2019s Clinic No. 13\u201d a document proving the identity of the representative and a duly executed power of attorney containing the representative\u2019s powers to submit applications to state bodies, organizations for obtaining data and information, including those constituting a secret protected by law, in relation to Alexandra Cheropoulou.<\/em>\u00a0Unfortunately, the power of attorney you submitted earlier does not contain these powers<\/em>.<\/p>\n\n\n\n

In accordance with Article 20, Clause 1 of Law of the Republic of Belarus No. 300-\u0417 of July 18, 2011, \u201cOn Appeals of Citizens and Legal Entities\u201d, the decision of the clinic administration can be appealed to the Health Committee of the Minsk City Executive Committee in the manner prescribed by law.<\/em><\/p>\n\n\n\n
